- Login to access your worklist, task history, and account settings

If you don’t have access to CoverMyMeds, you can still get all the advantages of submitting PA requests electronically. Simply use our secure PromptPA portal.

Submit your PA request by fax
While ePA is preferred (and required by certain states), we do offer prior authorization by fax. Simply download the appropriate request form below.
Tips for fax submission:
- Check the patient’s BIN to determine which form to download
- One member per request
- One prescription per request
- Attach relevant supporting clinical documentation
- Check form for accuracy and completeness before sending

How to appeal a PA decision
If your PA request is denied, members and prescribers have the right to appeal.
File your appeal in writing within 180 days of the decision notice- Include all required information and supporting clinical documentation
- Fax to 800-476-2691 or mail to LucyRx, 7815 N Palm Ave., Suite 400, Fresno, CA 93711
Appeal decision timing:
Standard (15 days): Decision provided within 15 days of receiving your appeal.
Fast (48 hours): If a patient’s health is at risk, you may request an expedited appeal. Decision provided within 48 hours.
Important: When you submit PA documentation showing urgency due to patient health, it automatically qualifies for a Fast appeal.
