Tips for fax or mail submission:

  1. One member per request
  2. One prescription per request
  3. Attach relevant supporting clinical documentation
  4. Check form for accuracy and completeness before sending

Prior Authorization FAQs

We’re here to help make PA requests easier for prescribers, patients, and caregivers. Below are answers to FAQs about submitting, tracking, and managing PAs. Whether you’re using ePA, fax, or mail, we’ve got you covered.

If your PA request is denied, members and prescribers have the right to appeal.

  • File your appeal in writing within 180 days of the decision notice
  • Include all required information and supporting clinical documentation
  • Fax to 800-476-2691 or mail to LucyRx, 7815 N Palm Ave., Suite 400, Fresno, CA 93711
Appeal timing:

Standard (15 days): Decision provided within 15 days of receiving your appeal.
Fast (48 hours): If a patient’s health is at risk, you may request an expedited appeal. Decision provided within 48 hours.

Important: When you submit PA documentation showing urgency due to patient health, it automatically qualifies for a Fast appeal.

To ensure your appeal is reviewed efficiently, include:

  • Patient full name, address, and ID number
  • Reason for appeal
  • Supporting documentation (e.g., medical reports, prescriber letters, or other clinical information)

Yes. If the plan denies your appeal, you may request an independent, external review on behalf of your patient within 120 days of the notice of the adverse appeal decision. We will send instructions on how to request an independent review or you may contact us directly.
